We are asking you to take the Setting New Paths Survey. This will take 10-20 minutes of your time.

Participate in Future Research

Research in people of African Descent is important to improve heart health in the community. The researchers would like permission to contact you at a future date to participate in heart health related research. This may include a project that links the results of this survey with other health database. Giving us permission to contact you does does not mean you have to participate in any future research. You contact information will not be given to the public.

Participate in Community Projects

The researcher are planning many heart health community events. These will take place in the Preston Township and will focus on different ways to improve heart health. If you would like to be contacted about these events let us know in the survey!

Permission to contact you about future research and community events is completely optional.

What's in it for me?

By participating in the survey you will be entered into a draw that includes several pre-paid credit cards and other community prizes. You must meet eligibility criteria in order to receive the prize.
